Resumen

En los últimos años se han descubierto nuevos aspectos fisiológicos del sistema renina-angiotensina que podrían producir cambios en las estrategias terapéuticas seguidas hasta ahora para conseguir su inhibición. Especialmente, el descubrimiento de la existencia de un sistema renina-angiotensina intracelular funcional ha permitido comprender muchos aspectos de la fisiopatología del remodelado cardiaco y puede ayudar a explicar la eficacia sólo parcial de los inhibidores de la enzima de conversión de la angiotensina y de los bloqueadores del receptor de angiotensina, así como la superioridad de los inhibidores directos de la renina, en determinadas condiciones patológicas asociadas con la enfermedad cardiovascular.
